How to Interpret ECG Tracings with Arrhythmia


This very brief chapter may in fact require a lot of time if the student reads the chapter slowly trying to remember all the concepts previously commented upon.


When a recording with arrhythmia appears in clinical practice, the recommendations described below should be used to reach the correct diagnosis. The information in Chapters 10 to 13 is to be used in this process.



  1. Confirm the presence of atrial activity and determine the type of rhythm (sinus or ectopic), rate, morphology, and location of atrial activity in the RR cycle.
  2. Analyze the QRS complexes: rate and morphology.
  3. Analyze the AV relationship and identify normal or retrograde conduction (Fig. 10.2).
  4. Analyze the premature complexes (extrasystoles, parasystoles, sinus capture) and determine the presence of repeating complexes (automatic or reentrant).
  5. Analyze pauses. Identify decreased automaticity, blocks, or concealed atrial bigeminy.
  6. Analyze the delayed sinus complexes and the complexes and escape rhythms.
  7. Analyze bigeminal rhythm.
