TIMI frame count: is it an adequate measure for embolization during stenting in primary PCI?

Background and Aim

TIMI frame count (TFC) is frequently used as an indicator for successful intervention during primary PCI. It is suggested that distal embolization occurs in almost all primary PCIs. The aim of the study was to assess whether the TFC is markedly affected by distal embolization.

Methods

90 consecutive cases of primary PCI were reviewed. Included were only cases in which pre-stenting devices (balloons and /or thrombectomy) were used ( n = 64). TFC was obtained by conventional methodology for each individual injection before and after stenting. The average of these scores served as pre and post TFC. The Mac-Labs reports were reviewed for special events of worsening chest pain post stenting, markedly more pronounced ST elevation, severe bradycardia or AV-block, or hypotension requiring inotropes. The angiograms were evaluated for evidence of new embolization occurring during stenting.
