Seasonal Variation of Takotsubo Cardiomyopathy

We read the report “Comparison of Circadian Rhythm Patterns in Takotsubo Cardiomyopathy Versus ST-Segment Elevation Myocardial Infarction” by Sharkey et al. The investigators comprehensively evaluated the chronobiologic pattern of the presentation of Takotsubo cardiomyopathy (TTC), suggesting a peak occurred in the afternoon. They did not find any major difference in seasonal variation in TTC presentation. We recently published a report on the “Prevalence of Takotsubo Cardiomyopathy in the United States.” We analyzed 6,837 hospitalizations of TTC and found that TTC hospitalizations were more frequent in the summer months, particularly in July, and were less frequent during the cooler months. Other investigators have observed a similar summer peak in TTC presentation. This is opposite to the hospitalization trend observed for other acute cardiovascular diseases, such as acute myocardial infarction, hypertensive emergency, and so forth, for which a winter peak has been observed. The exact reason for such seasonal variation is not understood. We acknowledge our study had inherent limitations attributable to the administrative databases. We propose starting a national registry to understand this clinical entity better.
