Duplex Assessment of an Arterial Stenosis


The raison d’etre in the Duplex assessment of a vascular stenosis is to decide if it is critical or not. A critical stenosis is one that meets parameters that are considered haemodynamically significant (and hence intervention should be strongly considered). Duplex findings will help assess the need for intervention and its planning in addition to complementing other angiographic studies.


In addition, the lesion should be assessed in combination with the simultaneous assessment of proximal velocities (calculate the velocity ratio), distal waveform (effects on distal flow velocities), the number of lesions and their lengths.


Objectives of Duplex Assessment



  • Identify pathology (stenosis, occlusion, aneurysm).
  • Localise (the lesion(s)).
  • Grade (the severity of the lesion(s)).

Grading and Assessment



  1. Peak systolic velocities*.
  2. Velocity ratio across the lesion*.
  3. Waveform shape (flow pattern)*.
  4. In-flow and run-off (and their velocities).
  5. Flow direction.
  6. Vessel size.
  7. Estimated narrowing (on grey-scale).
  8. Diastolic velocities (more relevant low-resistance tissues).
  9. Plaque morphology and intimal thickness.
  10. Flow quality (disturbance).
