Cardiac conduction disturbances after percutaneous balloon aortic valvuloplasty




Background


Disturbances in atrioventricular conduction and further need for permanent pacemaker are well-recognized complications after transcatheter aortic valve implantation (TAVI). Percutaneous balloon aortic valvuloplasty (PBAV) is considered as an integral step in the procedure of percutaneous aortic valve replacement. However, the effect of the PBAV on atrioventricular disturbances has not been elucidated. We therefore sought to evaluate the incidence and type of electrocardiogram (EKG) changes associated with PBAV.




Methods


We studied 329 consecutive patients with symptomatic severe aortic stenosis who underwent PBAV. The standard 12-lead EKGs were analyzed before and after the procedure. Heart rate, QRS width, PR and QT interval were measured, and the presence of atrioventricular block was documented.

Results


There were no change in PR interval (181.2±48.5 vs. 181.3±41.9, P =.017) and a slight increase in QRS duration (113.9±33.5 vs. 119.6±54.6, P <.001). New LBBB was found in 2 patients (0.6%) and new LAHB in 11 patients (3.3%). No new RBBB or LPHB was documented. AF was seen in three patients (0.9%) after the procedure. Permanent pacemaker was required only in three patients (0.9%) after the PBAV ( Table 1 ).
