The treatment of VVs is largely conservative, mainly consisting of patient reassurance in uncomplicated VVs without signs of venous insufficiency. In mild venous insufficiency, compression stockings usually suffice but in significant venous insufficiency, surgery is usually indicated (as well as stockings postoperatively). Surgery for VVs has been revolutionised in the last decade or so.


Treatment


Conservative


The principles of conservative management are first to reduce the duration of high pressure. So, first, advise the patient to:



  • avoid standing for too long (stops gravity from creating a hydrostatic column of blood)
  • put their feet up when resting.

Second, fight pressure with pressure, which means compression stockings:



  • Patients with VVs without ulceration can wear Class II (25–35 mmHg) BK graduated compression stockings. They are graduated from high pressure distally to lower pressure proximally
  • When ulceration has developed, patients should have four-layer compression bandaging, which usually results in healing in approximately 6 weeks.
