PP-139 A Rare Complication of Chest Trauma; Saccular Aortic Aneurysm Induced by Blunt Chest Trauma and Successful Endovascular Treatment




Introduction


An aortic aneurysm is defined as an enlargement of the aorta to greater than 1.5 times normal size. Fusiform and saccular aneurysms are the types according to shape. Blunt trauma to the chest, such as traffic accident may cause aortic dissection or rupture. However, a saccular aneurysm is rare after traffic accident as a pedestrian injury. We aimed to present a case with saccular aneurysm in the proximal part of the thoracic aorta after pedestrian injury and successful treatment with endovascular stent.




Case Description


A 19-year-old male had a car accident while crossing the road as a pedestrian. He was referred to emergency department and urgent computerized tomography(CT) was performed due to chest and back pain. CT showed a suspicious dissection of the proximal of the thoracic aorta. He was referred to our clinic for further treatment. We decided to perform multislice CT for the exact diagnosis. Multislice CT demonstrated a saccular aneurysmand intimal dissection flap located 2 cm distally from the left subclavian artery. After the diagnosis endovascular treatment of the aneurysm was decided. Medtronic VALIANT 28×100 endovascular stent was deployed from the right femoral artery in the angiography laboratory.. Figure 1 demonstrates the pre-procedural and post-stent images. Patient was successfully discharged 3 days after the treatment.
