PP-035 T Wave Surge on Precordial Leads During Treadmill Test may Indicate the Presence of Myocardial Bridging on LAD Artery in Young Subjects




Aim


Various types of ST segment or T wave changes on treadmill test (TT) which could be associated with presence of coronary artery disease have been described previously. Myocardial bridging (MB) is a common variant of coronary artery anomalies but it may lead ischemia upon extraordinary or prolonged type exercises especially in young subjects. We observed that a novel type of T wave variation could be related with the presence of MB on LAD artery. We defined it as T wave surge which peaks up to 2 times of baseline T wave height. We observed those T wave surge developed on last stage of TT during the 95% of age predicted maximal heart rate and continued during the recovery period in three young patients who was diagnosed as having MB on LAD artery via MSCT Angiography.




Case 1


A 24 years old male presented with dyspnea on effort and had T wave surge up to 2.4 times of its baseline height initiated at stage 4 and continued to recovery phase of TT. MSCTA revealed MB on the proximal segment of LAD artery with mild luminal narrowing.
