OP-196 The Risk of Future Cardiovascular Events and Requirement of Coronary Artery Revascularization in Dialysis Patients with Coronary Artery Calcific Disease




Objective


Our aim of this study was to investigate the relationships between coronary artery calcific (CAC) score levels and future cardiovascular events which requirement surgery or percutenous coronary interventions in asymptomatic hemodialyses dependent patients(HDDP).




Methods


A multydetector computed tomography (MDCT) for CAC scores has been performed in our 307 HDDPs.We followed the patients for symptoms of coronary artery disease which requirement of revascularisation and major adverse cardiac and cerebrovascular events (MACCE).

Results


CAC scoring more then zero has been detected in 107 asymptomatic patients.CAC scores were more than 300 in those 86 patients.In three years period,19 patients died (17.7%).13 patients died due to ischemic cardiac event (12.1%).Three patients died because of cerebrovascular event(2.8%).Total 67 patients required coronary revascularisation.PCI and/or balloon angioplasty has been performed in 38 patients with CAC sores was lower than 600.In the remaining 29 patients CABG has been performed.In this group,the mean CAC scores were more than 1000.4 patients (13.7%) in CABG group and one patient (2.6%) in PCI group died (P<0.0001).The mean hospitalisation time was 14±4.2 d and 9±3.4 d in CABG and PCI group,respectively (P<0.002).Post-intervention MACCEs have been detected in three patients in CABG group (10.3%).one patient suffered from MACCE after PCI(2.6%) (P=0.001).Ten patients(27%) from PCI group and two patients(8%) from CABG group required repeat revascularisation(P<0.001).Total survival rates in PCI group at one, two, and three years were as follows: 56.9%, 44%, and 21%, respectively. In CABG group survival rate was calculated for the first, second and third year was 64.5%, 49% and 34.3.
