The following information was omitted and should have appeared on the first page of the article in the Methods section: The CADICE study (recruitment of the patients and data entry) was conducted at the Clinica Cardiovascular Santa Maria (Medellin, Colombia) between January 2006 to May 2007. Statistical analysis and reporting included in the manuscript was performed in collaboration between the Corbic Research Institute (Envigado, Colombia) and the Skirball Center for Cardiovascular Research (Orangeburg, NY).
