CLL and Thoracic Health: Understanding Treatment for CLL and Its Systemic Implications

Chronic lymphocytic leukemia (CLL) is a form of blood cancer, found in the B lymphocytes or white blood cells that are part of the body’s immune system. The cancer is generally slow growing, and in its early stages, CLL does not produce any symptoms and requires no treatment. However, as time passes, the abnormal lymphocytes can spread throughout the body and collect in the lymph nodes (glands) or in tissues in the chest. As CLL develops, it can affect many aspects of a person’s health and must be carefully monitored as it affects the thoracic region and can cause a range of respiratory symptoms.

This article explores the link between CLL, thoracic health, and respiratory symptoms, including how they may relate to the disease and its CLL treatment. It also reviews current treatment approaches for CLL and discusses factors that may impact individual patient experiences during treatment.

How CLL Can Affect Thoracic Health

As with other parts of the body, enlarged lymph nodes in the chest can on occasion cause symptoms as they press on surrounding structures. Usually however, CLL-related chest symptoms are not caused by CLL itself but by a treatable infection.

Infections, especially of the respiratory tract, are of major concern in patients with CLL as their immune system is already severely affected by the leukemic process. Symptomatic episodes such as persistent coughs or shortness of breath or extreme fatigue may require treatment of underlying infections. The cause of symptoms in the thoracic region for individuals with CLL can be due to many factors including the CLL itself or other, totally separate health problems. Even the effects of CLL treatment can sometimes cause symptoms in the chest region.

Lymph Node Enlargement and the Chest

Lymph nodes with CLL involved in them are in various parts of the body, including in the thoracic region. The CLL involved lymph nodes can become enlarged in the chest area.

Whether enlarged lymph nodes in the chest are of clinical significance depends on a variety of factors, including the location, size and rate of increase in size of the lymph nodes as well as the presence of symptoms. The lymph nodes are evaluated during follow-up of CLL patients and changes in the lymph nodes are assessed by physical examination, measurement of blood parameters and by imaging studies as indicated.

Enlarged lymph nodes within the chest may cause symptoms as they push against surrounding structures, but not all cases need to be treated. It will depend on severity of symptoms and general progression of disease.

Respiratory Symptoms Should Be Evaluated

When a person with CLL (as well as their family member/carers) notice new, persistent respiratory symptoms such as cough, shortness of breath, chest discomfort, fever and/or repeated episodes of infection which affect breathing, they should report these symptoms to the healthcare team and seek appropriate evaluation/treatment of these symptoms as for any other cancer patient. These symptoms should not be assumed to be due to CLL, and proper evaluation of all possible causes needs to take place.

In CLL, an individual’s immune system may be impaired in certain areas and as a result he or she can get infected more easily by typical respiratory pathogens. As a result of a CLL related infection a person with CLL may develop typical symptoms of a respiratory infection, which in turn have to be evaluated by a physician.

The observation of respiratory symptoms in patients with CLL should always be carefully evaluated for causes other than progression of the disease. Symptoms can be caused by an infection, by pressure on neighboring structures by enlarged lymph nodes, by side effects of medication and by other circumstances.

When CLL Treatment May Be Needed

A CLL diagnosis does not necessarily mean that treatment is required. Most patients with early-stage CLL (asymptomatic) are managed with active surveillance (‘watch and wait’). The healthcare professional would monitor blood counts, size of lymph nodes, spleen, as well as symptoms over time. Treatments are usually indicated when CLL becomes active or in particular when CLL causes complications.

Watchful waiting is appropriate for patients with CLL, even those who have been diagnosed with early-stage asymptomatic CLL. They need to be monitored for indicators of progression or complications such as an increase in lymphocytes in the blood, a rise in lymph node or spleen size, development of anemia or low platelet count, symptoms of infection, unintentional weight loss, fever of unknown origin, night sweats and fatigue.

Reasons for considering treatment can include progressive lymph node enlargement, worsening anemia or thrombocytopenia, enlargement of the spleen, persistent fever, night sweats, unexplained weight loss, or significant disease-related fatigue. An increasing lymphocyte count is considered within the overall evaluation, but an increasing number of lymphocytes itself does not necessarily require treatment.

Modern Treatment for CLL

Today there are targeted therapies available as a treatment option for CLL apart from chemotherapy. One such category of CLL drugs are BTK (Bruton’s Tyrosine Kinase) inhibitors. These medicines are given orally and work by targeting Bruton’s Tyrosine Kinase, a critical component in B-cell receptor signaling found in normal B-cells as well as the CLL B-cells. BTK signaling is necessary for the survival of CLL cells. Three medicines that inhibit BTK are ibrutinib, acalabrutinib, and zanubrutinib.

A second class of targeted therapies is the BCL-2 inhibitors. Many CLL cells depend on the BCL-2 protein for their resistance to programmed cell death (apoptosis). Inhibition of BCL-2 leads to CLL cell death and has been evaluated as a treatment option for some CLL patients. They can be given in different ways depending on individual clinical circumstances and sometimes patients are on continuing treatment whilst on treatment regimens for set periods of time.

Considering Systemic Health

Disease is systemic, meaning it can affect other parts of the body in addition to where symptoms are first observed. A comprehensive assessment of a patient’s health includes consideration of all factors when determining the best course of treatment for their CLL.

Factors that affect systemic health of a CLL patient can include blood counts, immune status, renal function, liver function, cardiac status, other medical problems, past treatment, and interactions with current medications. Potential toxicities must also be considered and sometimes monitored by parameters such as blood pressure, cardiac function, blood counts, renal function, liver function and the occurrence of infections. For example, treatment can be initiated at a lower dose and increased in a stepwise fashion to allow for the CLL cell destruction to be managed to prevent tumor lysis syndrome.

Infection Prevention and Thoracic Health

In addition to attempts to tackle the CLL itself, managing the immune system is key to long-term management of CLL and prevention of infections such as respiratory tract infections. The CLL patient may discuss vaccination and other preventive measures with the healthcare provider. Individuals with CLL and newly developed symptoms of infection should receive prompt evaluation of worsening symptoms. In some cases, symptoms may worsen rapidly and need immediate medical attention. Changes in respiratory health should be discussed with the healthcare provider.

Individualized Treatment Decisions

There is no single treatment for CLL and individualized treatment decisions are required based on disease stage, molecular characteristics, previous treatment(s), patient health status and associated co-morbidities, potential treatment side effects and drug interactions as well as individual patient preference.

A physician must determine whether the symptoms such as lymph node enlargement, infection, drug toxicity or other health problems are affecting the CLL patient. The patient should discuss all symptoms such as shortness of breath or chest discomfort with his/her physician.

Looking Ahead

Many research efforts are currently directed at the further elucidation of CLL to open new ways of specific tumor therapy. Targeted drugs, combination therapy, and time-limited therapy are among the strategies being studied and used in CLL management.

Understanding CLL’s systemic nature can empower people living with the disease to recognize potential symptoms affecting different parts of the body, be it respiratory problems, chest discomfort or pain, fever or repeated infections. The key to correct treatment is timely and accurate evaluation of new or persistent CLL-related symptoms by healthcare providers.

Individualized treatment is the cornerstone in managing CLL and therefore a specialized doctor like a Hematologist/Oncologist needs to assess all aspects of a person’s illness and decide whether to monitor, treat with a single targeted therapy or combine it with others for a certain duration etc.
