A 65-year-old man had a bioprosthetic mitral valve (MV) replacement 6 years ago. He now presents with worsening congestive heart failure (CHF). A transesophageal echocardiogram is done to evaluate the cause of the CHF and the bioprosthetic MV function and morphology (Videos 49-1 and 49-2).



QUESTION 1. The cause of the CHF is:
